Important Reminders from the School Nurses
Hello and welcome to the (almost) start of the school year. Please see below for important information. We look forward to a successful, safe and healthy school year ahead!
If your child(ren) will be taking any medication at school, we will need a doctor’s order and a signed parent permission form, along with the medication in a pharmacy labeled container. If your child(ren) have asthma or food allergies, we need new inhalers and epinephrine along with the appropriate paperwork. All forms need to be updated yearly. There are blank careplans for health issues such as seizures, food allergies and asthma on our website. At the start of the school year, you will receive a green health card that needs to be completed and your child(ren) should return it to school. Your attention to this will provide us with up to date health information, and there is also a section to sign permission for your child(ren) to receive over the counter medication from the school nurse (tylenol, ibuprofen, cough drops, hydrocortisone cream, and benadryl for emergencies). Please note, we do not have allergy medication. This is a frequent request by students, but we need a doctor’s order, a parent permission form, and the medication from home for medications outside of the 5 medications listed in parentheses.
Massachusetts State Law requires documentation of a doctor’s physical examination upon entering school and every three years thereafter. We request these prior to the start of 4th and 7th grades in Brookline elementary schools, an exam dated within a year of entry into 4th or 7th is acceptable. Thank you to those who have submitted physicals already, and if you have not already done so please send your child(ren)’s physical exam form to us.
